Diseases caused by dirty environment remains a major cause of death in Indonesia. Even in those infants and toddlers, environment-based diseases contributed over 80% of the illnesses suffered by infants and toddlers. The Indonesian government has a very strong commitment to achieving the Millennium Development Goals (MDGs), namely the declining number of people who do not have access to drinking water and basic sanitation by 50% in 2015. One government program that emphasizes the improvement of environmental sanitation in rural areas is WSLIC-3/PAMSIMAS program, one of the programs and real actions of government (central and local) with World Bank support, to improve drinking water supply, sanitation, and improving community health status especially in reducing diarrheal diseases and other diseases transmitted through water and the environment. The whole process of planning such as the selection PAMSIMAS water requirements and implementation of activities include active participation of society, women are no exception. This is a manifestation of community needs for drinking water and sanitation facilities, so hopefully that means waking maintained and managed by the community. This research used a mixture of quantitative research methods with qualitative research methods. Given the wide scope of territory PAMSIMAS program, then this study will limit his study area in the hamlet Kalisari and Tubansari, Margoyoso Village, District Salaman, Magelang regency. From Hamlet Kalisari and Tubansari, each will be drawn at random 160 people respondent. Socio-cultural factors affecting community participation in drinking water programs and community-based sanitation (PAMSIMAS) are education, public health efforts, community norms, knowledge of sanitation, and perceptions about PAMSIMAS. Public participation in the program PAMSIMAS participation in an advanced stage of implementation which are mostly done by people, that is equal to 53.60%. This is because people prefer to work directly rather than participate in a meeting thinking that can only sit, silent, thinking, and listening. Communities also considers that participation in the implementation phase is the actual participation because they work directly together for a purpose. Communities have been behaving good sanitation, such as for the needs of cooking, washing, and bathing them also have been using clean water. For the needs of defecation gradually reduced as society gradually has implemented a clean and healthy living behavior. Sanitation behavior is learned through health education. Participation in PAMSIMAS also become more influential because people can learn and practice with the means PAMSIMAS.
